As health care delivery in the United States continues to evolve, either through mandates, improved technologies and training, or other drivers, nurses remain at the forefront in facilitating the success of new initiatives. In 2010, the Institute of Medicine formed a committee of experts to address the following question:

“What roles can nursing assume to address the increasing demand for safe, high-quality, and effective health care services?”

In the final report of the committee, The Future of Nursing: Leading Change, Advancing Health, the authors stated:

This report offers recommendations that collectively serve as a blueprint to

(1) ensure that nurses can practice to the full extent of their education and training,

(2) improve nursing education,

(3) provide opportunities for nurses to assume leadership positions. Also, to serve as full partners in health care redesign and improvement efforts, and

(4) improve data collection for workforce planning and policy making. (p. 10)

For this Discussion, you will focus on the research recommendations presented by the committee concerning the role of nurses as leaders. The committee members believe that answers to these research questions are needed to advance the profession of nursing. They also believe the answers will further expand their role in health care reform and improvement.

ExcerptThe Future of Nursing examines how nurses’ roles, responsibilities, and education should change dramatically in order to meet the increased demand for care that health care reform will generate and to advance improvements in America’s increasingly complex health system.

With a workforce of more than 3 million, nurses are the single largest segment of the health care workforce. Additionally, they devote the most time to patient care as a profession. Nurses therefore possess unique insights and abilities to collaborate with other health care professionals to improve the quality and safety of care, as envisioned in the recently enacted Affordable Care Act (ACA).

Nurses should collaborate fully with other health professionals and take the lead in redesigning care in the United States. To ensure that its members are adequately prepared, the profession should establish residency programs for nurses, increase the percentage of nurses with a bachelor’s degree to 80% by 2020, and double the number of nurses pursuing doctorates. Additionally, regulatory and institutional barriers should be removed, including limitations on nurses’ scope of practice, so that the health system can fully benefit from nurses’ training, skills, and knowledge in patient care.

The Institute of Medicine makes recommendations in this book for an action-oriented blueprint for nursing’s future.

The decade ahead will put the nearly 4 million nurses in the United States to new and complex tests. Nursing professionals live and work at the crossroads of health, education, and community. Nurses practice in a variety of settings and at a variety of professional levels. They are frequently the first and most frequent point of contact for people seeking care from all backgrounds and experiences, and they comprise the largest of the health care professions.

A nation cannot thrive fully until everyone – regardless of race, location, or income – can live their healthiest possible lives, and assisting people in living their healthiest possible lives is and has always been an essential role of nurses. Nurses are critical to achieving health equity, but they require comprehensive education, supportive work environments, and autonomy. As a result, at the request of the Robert Wood Johnson Foundation and on behalf of the National Academy of Medicine, an ad hoc committee convened by the National Academies of Sciences, Engineering, and Medicine conducted a study aimed at envisioning and charting a path forward for the nursing profession in order to contribute to the reduction of inequities in people’s ability to achieve their full health potential. The ultimate goal is to strengthen nursing capacity and expertise in order to achieve health equity in the United States. By leveraging these characteristics, nursing can contribute significantly to the creation and maintenance of equitable public health and health care systems that work for everyone.

The Future of Nursing 2020-2030: Charting a Path to Health Equity examines how nurses can continue to work to reduce health disparities and promote equity while controlling costs, leveraging technology, and providing patient- and family-centered care into 2030. This work builds on the foundation laid out in the 2011 report The Future of Nursing: Leading Change, Advancing Health.
