PHI 413 Topic 3 Discussion Question One
Principlism, especially in the context of bioethics in the United States, has often been critiqued for raising the principle of autonomy to the highest place, such that it trumps all other principles or values. How would you rank the importance of each of the four principles? How do you believe they would be ordered in the context of the Christian biblical narrative? Refer to the topic overview and other topic study materials in your response.

I believe that bioethics is a very difficult topic not only to explain but also to understands as each individual has his or her own ideas, beliefs, culture, values, religion, among others. Bioethics and ethics can influence behaviors among healthcare workers, enhancing a respectful, fair, free-from-harm care to their patients. Justice, autonomy, beneficence and no-maleficence are the four main principles followed by healthcare worker. The US is driven by these four principles also known as Principlism. I believe that when applying these principles, it is imperative that we apply each one of them without ranking one as more important than the other. Each principle complements each other in order to provide excellent patient care. Ensuring that all the principles are ranked equally also help in assessing whether an action taken towards the care of a patient, work-related and among others, meets the criteria for it to be good, or not. In addition, it is very hard for me to believe that autonomy should be ranked the highest principle because as mentioned before, our society is very diversified with different point of view. Ranking autonomy as the highest could be challenging for the patients, coworkers, family member and even us as nurse caring for patients that can make choices that may compromise their health.

When referring to the Christian biblical narrative, justice was the first and highest ethical value because it ensures the correct way to act. Later on, beneficence and non-maleficence were added and ranked as the same level as justice in ethical values of Christendom. Autonomy was then added last as the fourth principle, as awareness of the differences in human beings were acknowledged and the freedom of choice was opened.

As mentioned above in the instruction, autonomy is raised “to the highest place, such that it trumps all other principles or values”. I am sure the values that we a share in our nation values independence or autonomy more than anything and that stance also translates into autonomy in other parts of our lives. Often times, our belief in enjoying the utmost autonomy has as stated trumps everything else including the three principles which are mostly seen as parts and parcels of bioethics. These four are “Principleism or also referred to as the “four principle approach,” is part of the framework of bioethics. The four ethical principles include autonomy, beneficence, nonmaleficence, and justice” (Grand Canyon University,2019).According to Ostman et al. (2019) “Ethical values are essential for any healthcare provider. Ethics comes from the Greek word “ethos,” meaning character. Ethical values are universal rules of conduct that provide a practical basis for identifying what kinds of actions, intentions, and motives are valued”. There is no doubt about the role these four principles paly in healthcare when it comes to deciding the most ethically challenging cases. As mentioned in the reference resource , some contexts make ethical decisions much harder than others especially when ,as healthcare workers, we encounter patients who are close to dying and have no expressed plan for their care in times like these.
Some argue that can be ranked as more important than another. According Lawrence DJ (2007) “The four principles that form the core of modern bioethics discussion include autonomy, beneficence, nonmaleficence and justice. The originators of these principles claim that none is more important than another, yet challenges have been laid against these principles on that basis as well as on other areas of disagreement”. Therefore it is very difficult to rank them in the order of importance as they all are important. In the instance mentioned in the reference , autonomy can’t be placed on the same scale as others as the patient can’t make the decisions due as Mrs. Jones is incapacitated. What is more relevant in her case is the practice of beneficence , preventing harm.
Therefore, it is my belief that the Biblical narrative also palaces as much importance on all four depending on the situation.
