Phase 1: Preparing for CPE
Meet with your preceptor or SME to review the requirements for your CPE. Discuss the need for a HIP in your former or current organization. Strategically plan the tasks and timeline needed to complete your CPE. You should collaborate with colleagues from your former or current organization, preceptor or SME, and faculty to complete an affinity analysis, impact analysis and a strengths, weaknesses, opportunities, and threats (SWOT) analysis to determine organizational need and readiness for a HIP.
Work with your preceptor or SME to identify at least three subject matter experts or colleagues from your current or former organization (e.g., nurse leaders, informatics experts, system users, etc.) who may be willing to participate in a collaborative affinity analysis, impact analysis, and SWOT. Consult with these individuals, preceptor or SME, and faculty to gain an enhanced understanding of their roles and responsibilities in initiating, planning, implementing, and evaluating a healthcare improvement project.

Consult with experts in the field or colleagues from your former or current organization, preceptor or SME, and faculty to determine the need for a HIP by doing an affinity analysis. You should capture, cluster, categorize, and prioritize the needs ideas. Use the results of your affinity analysis to determine which need will be the focus of your HIP. Strategically plan to address the need that emerged from your affinity analysis by developing a set of goals, objectives, and recommendations for a HIP. This should include a justification for the need in context of your target population and project setting. Also include the method used to reach group consensus.
Conduct an impact analysis using the “Impact Analysis Template” to further examine the feasibility of implementing a HIP. The analysis should include the following:
Two benefit and risk categories
Two benefits for each category and two risks for each category
A rating for each benefit and each risk
A calculation of the total benefits score and the total risks score
The result of the impact ratio calculation
Conduct a SWOT analysis using the “SWOT Analysis Template” to determine the strengths, weaknesses, opportunities, and threats of the organization where you will implement your healthcare improvement project. The analysis should include the following:

method of quantifying the impact that the benefits will have. Give a “1” rating for Low

INTRODUCTION
Note: You must have completed and passed the performance assessments for D155: Leading with Personal Mastery IOM1 Task 1 and 2 prior to beginning this performance assessment.

 

After your first course, the remaining specialty courses through your capstone will have a summative assessment that consists of an authentic performance assessment that scaffolds the tasks of the healthcare improvement project (HIP) through the project management lifecycle phases of project initiation, planning, implementation, and evaluation. Each performance assessment will focus on aspects of the proposal you will develop for a healthcare improvement project using a real-world approach to improving healthcare. Each phase will be described in different sections of a Healthcare Improvement Project (HIP) paper.

 

This task is focused on the knowledge you gained and the deliverables you completed to initiate the proposed HIP using information from a former or current organization. You will describe the collaborative process used to complete the business case analysis by determining the need, feasibility, and organizational readiness for your HIP. As the nursing leader developing the business case analysis in this initiation phase, your end goal is to justify the project.

 

This task requires the submission of your entire HIP paper template including the following section(s) of your HIP paper which you will be developing in this performance assessment:

• Stakeholder Identification”

• Needs Assessment using Affinity Analysis”

• Feasibility Assessment”

• Organizational Readiness and SWOT Analysis”

• SMART+C Goal and Outcome Statement

 

While you must submit your entire paper, you will only be graded on the project initiation section and related appendices. The following templates should be completed and submitted as appendices to your HIP paper:

• Impact Analysis Template (Appendix B)

• SWOT Analysis Template (Appendix C)

• SMART+C Goal Worksheet (Appendix D)

 

During your clinical practice experience (CPE), you will have an opportunity to identify project stakeholders (subject matter experts, colleagues from a former or current organization, faculty and other nursing professionals) who have agreed to collaborate with you during this phase of your project. You will consult with appropriate stakeholders to complete and submit the “Impact Analysis Template” and “SWOT Analysis Template”. These deliverables will be evaluated in your CPE for completion, not quality. When you submit these deliverables as appendices with your HIP paper in this task, they will be evaluated for content and quality of completion so please check your work to make sure it meets the requirements for your HIP paper. CPE provides you an opportunity to practice and improve these items before including them in your Task 1 performance assessment.

REQUIREMENTS
Your submission must be your original work. No more than a combined total of 30% of the submission and no more than a 10% match to any one individual source can be directly quoted or closely paraphrased from sources, even if cited correctly. The originality report that is provided when you submit your task can be used as a guide.

You must use the rubric to direct the creation of your submission because it provides detailed criteria that will be used to evaluate your work. Each requirement below may be evaluated by more than one rubric aspect. The rubric aspect titles may contain hyperlinks to relevant portions of the course.

Tasks may not be submitted as cloud links, such as links to Google Docs, Google Slides, OneDrive, etc., unless specified in the task requirements. All other submissions must be file types that are uploaded and submitted as attachments (e.g., .docx, .pdf, .ppt).

Complete the “Healthcare Improvement Project Initiation” section of your healthcare improvement project (HIP) paper by doing the following:

Stakeholder Identification

A. Summarize the process you used to identify three individuals who will contribute to the success of your project proposal by becoming members of your project team. Include the following information in your summary:

• the technique you use to identify the three individuals

• a brief description of the three individual’s experience or expertise

• the role they will have in supporting the proposed healthcare improvement project (HIP)
