Social determinants of health entail various conditions prevalent in an individual’s environment, including age, religious beliefs, play, work, learning environment, residence, and even birth, which impact an individual’s quality of life and functionality (Katz et al., 2018). Therefore, it is vital to address social determinants of health to help reduce health disparities and improve people’s health. In family nursing, the social determinant of the health of a particular family can be achieved through an elaborate family assessment. Having accomplished assessment of a family composed of five members in the family health assessment part one, this part two aims to discuss the family’s social determinant of health in detail. In addition, age-appropriate screening for every family member, a health model to assist in action plan creation, and family-centred health promotion will all be explored.
Description of the SDOH That Affect the Family Health Status
From the family health assessment done, a range of social determinants was identified that have a bearing on the current health status of the family, both the ones that positively affect their health and the ones that negatively impact it. The family’s education and literacy levels can best be described as good since two of the children are in high school while one of them is in elementary school. Moreover, the parents are working-class, and both have college degrees. The commendable level of education and literacy in the family is one of the reasons that they have a relatively better health status, as the parents are proactive in both the choice of meals and physical activity undertaken in the family. Indeed, high literacy levels and education have been proven to impact individual’s or families’ health status positively (Northwood et al., 2018)

In a 750-1,000 word paper, create a plan of action to incorporate health promotion strategies for this family. Include the following:
- Describe the SDOH that affect the family health status. What is the impact of these SDOH on the family? Discuss why these factors are prevalent for this family.
- Based on the information gathered through the family health assessment, recommend age-appropriate screenings for each family member. Provide support and rationale for your suggestions.
- Choose a health model to assist in creating a plan of action. Describe the model selected. Discuss the reasons why this health model is the best choice for this family. Provide rationale for your reasoning.
- Using the model, outline the steps for a family-centered health promotion. Include strategies for communication.
Cite at least three peer-reviewed or scholarly sources to complete this assignment. Sources should be published within the last 5 years and appropriate for the assignment criteria.
